Chaperone Waiver & Release of Liability

I agree to serve as a chaperone for the 1st grade field trip on March 25th. I understand that my role as a chaperone is to supervise and assist participants while ensuring their safety and well-being.

I acknowledge that participation in this event may involve certain risks. I agree to pay, either directly or through my own personal health and accident policy, all medical or hospital costs. I further agree to hold HQA-Spring, its Board of Trustees, administration and/or faculty harmless from all liability for any injury which said student may receive while participating in or while traveling to and from such event.

I certify that I am physically and mentally capable of performing chaperone duties and will comply with all event policies and guidelines.

By signing below, I confirm that I have carefully read and fully understand its contents. I am aware that this is a release of liability and signed it of my own free will.
